RESUMO

Liposarcomas are described as soft tissue sarcomas derived from adipose tissue. The finding of this tumor in the mandibular region is exceedingly rare. As of now, it has been described mainly in case reports and small series. A multidisciplinary approach is required to offer optimal treatment and may involve surgery, radiation and systemic therapies. Surgical repair of these defects represents a major challenge in oral and maxillofacial reconstructive surgery. We present the case of a 54-year-old man referred to our center with a progressively increasing mass in the anterior portion of the mandible. Biopsy revealed a well-differentiated myxoid liposarcoma. Resection of the tumor was performed with an additional primary reconstruction.

We present a rare case of primary caruncle basal cell carcinoma (BCC), a condition with limited occurrences. Our patient, an 80-year-old woman without prior ocular pathological history, presented a 2x2mm pedunculated blackish nodular lesion on the caruncle of her left eye, without local conjunctival or cutaneous involvement. Histological analysis following complete excision confirmed the presence of basal cell carcinoma within the caruncle. Over a span of 30 months, no recurrence has been observed. While scant cases are documented in the literature, we conducted a review of these instances. Despite its infrequent manifestation, this condition should be taken into account when evaluating caruncular tumors, given its tendency to invade the orbit. Complete excision with free surgical margins is the treatment of choice, and adjuvant radiotherapy or chemotherapy might be considered.

Genitourinary tract soft-tissue sarcomas are rare neoplasms with varied pathologic and clinical features. While some of these tumors may be aggressive high-grade malignancies, others are low grade with a relatively better prognosis. Given that the grade and extent of the disease are important prognostic factors in these tumors, timely diagnosis is crucial. Unfortunately, most imaging features of these malignancies are not pathognomonic, and various histologic subtypes do not manifest with typical classic imaging features. Therefore, reliable differentiation of the various histologic tumor types is not always possible based solely on the radiologic manifestations. Imaging findings need to be considered in the context of clinical history in corroboration with radiologic-pathologic correlation. The authors discuss the specific imaging and pathologic characteristics of various genitourinary tract soft-tissue sarcomas, emphasizing diagnostic difficulties and differential diagnoses. ©RSNA, 2024 Test Your Knowledge questions for this article are available in the supplemental material.

Bilateral posterior communicating (pComm) artery aneurysms represent only 2% of mirror intracranial aneurysms. Usually, these are surgically approached through bilateral craniotomies for clipping. We present the case of a 50-year-old female presenting with headache and horizontal diplopia. Neurological examination revealed a left oculomotor palsy, with no other neurological deficits. Imaging studies revealed bilateral aneurysmatic lesions in both internal carotid arteries (ICA). A conventional left pterional approach was planned in order to treat the symptomatic aneurysm, and, if deemed feasible, a contralateral clipping through the same approach. The procedure was performed in a hybrid operating room (HOR), performing an intraoperative digital subtraction angiography (DSA) and roadmapping assistance during dissection and clipping. Transoperatively, a post-fixed optic chiasm was identified, with a wide interoptic space, which allowed us to perform the contralateral clipping through a unilateral approach. This technique for clipping bilateral pComm aneurysms can be performed when the proper anatomical features are met.

BACKGROUND: Triatoma guasayana is considered an emerging vector of Chagas disease in the Southern Cone of South America. The presence of a triatomine population with brachypterous individuals, in which both wings are reduced, has recently been reported for this species. The aim of the present study was to determine if flight-related traits varied across populations, if these traits could explain differences in flight capacity across populations and if flight-related traits are associated with geographic and/or climatic variation. METHODS: The study involved 66 male T. guasayana specimens from 10 triatomine populations. Digital images of wing, head and pronotum were used to estimate linear and geometric morphometric variables. Variations in size and shape were analysed using one-way analysis of variance and canonical variate analysis (CVA), respectively. Mantel tests were applied to analyse the relationship between morphometric and geographic distances, and the association between size measurements was analysed using Pearson's correlation. We explored covariation between size and shape variables using partial least square analyses (PLS). The association of geographic and climatic variables with size measurements was tested using linear regression analyses. We performed PLS analyses for shape measurements. RESULTS: Wing size differed significantly across triatomine populations. The CVA showed that wing shape of the brachypterous population is well discriminated from that of the other populations. The Mantel test showed a positive and significant association between wing shape and geographic distances. The heads of the brachypterous population were significantly larger than those of the other populations. Similar to wing shape, the head shape of the brachypterous population was well discriminated from those of the other populations. Pronotum width did not show significant differences across populations. Geographic and climatic factors were associated with size and shape of both the wing and head, but not with pronotum width. CONCLUSIONS: Most of the traits related to flight dispersal varied across populations. Wing shape and head shape were found to be better markers for differentiated morphological variation across populations. Head measurements also varied in accordance with this condition. Geographic and climatic variables were associated with most of the flight-related traits.

Introducción: Las aplicaciones web actualmente son indispensables para el manejo de los datos en toda organización, permite al usuario acceder y utilizar la información desde cualquier parte del mundo. El uso de las aplicaciones web ha propiciado la agilización de varios procesos. Objetivo: Emplear una aplicación web desarrollada mediante software libre para el control y clasificación de historias clínicas por diferentes patologías del Centro Médico Salud y Vida de Ibarra, Ecuador. Método: Se aplicó una investigación de modalidad mixta cualitativa-cuantitativa la misma que permitió identificar necesidades reales de los pacientes y de los profesionales de esta casa de salud. El uso de la investigación histórica ayudó a conocer cada uno de los procesos que fueron tomados en cuenta en la generación de la historia clínica y cómo poder clasificarlas por patologías. Con ayuda de la investigación bibliográfica se logró recopilar definiciones necesarias para argumentar la base teórica del artículo; a través de la encuesta y entrevista se identificó la disconformidad de los pacientes por factores como el tiempo de espera, la deficiencia de búsqueda y peor aún no encontrarse registrados. Resultados: La agilidad en los procesos de registro de los pacientes y cada uno con su respectiva historia clínica permite que en las próximas citas los especialistas continúen con su diagnóstico. Conclusiones: La aplicación web agiliza de manera efectiva los procesos relacionados a las historias clínicas, lo que contribuye al control y clasificación de dicha documentación.


Introduction: Web applications are currently essential for data management in any organization, they allow the user to access and use information from anywhere in the world. The use of web applications has led to the streamlining of several processes. Objective: To use a web application developed using free software for the control and classification of medical records for different pathologies in the Health and Life Medical Center of Ibarra, Ecuador. Method: A qualitative-quantitative mixed modality research was applied, which allowed us to identify the real needs of the patients and professionals of this health home. The use of historical research helped to know each of the processes that were taken into account in the generation of the clinical history and how to classify them by pathologies. With the help of bibliographic research, it was possible to compile the necessary definitions to argue the theoretical basis of the article; Through the survey and interview, patient dissatisfaction was identified due to factors such as waiting time, search deficiency and, even worse, not being registered. Results: The agility in the patient registration processes and each one with their respective medical history allows the specialists to continue with their diagnosis in the next appointments. Conclusions: The web application effectively streamlines the processes related to medical records, which contributes to the control and classification of said documentation.


Introdução: As aplicações web são atualmente essenciais para a gestão de dados em qualquer organização, pois permitem ao usuário acessar e utilizar informações de qualquer lugar do mundo. A utilização de aplicações web tem levado à agilização de diversos processos. Objetivo: Utilizar uma aplicação web desenvolvida em software livre para o controle e classificação de prontuários de diferentes patologias no Centro Médico Saúde e Vida de Ibarra, Equador. Método: Foi aplicada uma pesquisa quali-quantitativa de modalidade mista, que permitiu identificar as reais necessidades dos pacientes e profissionais desta casa de saúde. A utilização da pesquisa histórica ajudou a conhecer cada um dos processos que foram levados em consideração na geração da história clínica e como classificá-los por patologias. Com o auxílio da pesquisa bibliográfica foi possível compilar as definições necessárias para argumentar a base teórica do artigo; Por meio da pesquisa e entrevista, foi identificada a insatisfação dos pacientes devido a fatores como tempo de espera, deficiência na busca e, pior ainda, não estar cadastrado. Resultados: A agilidade nos processos de cadastramento de pacientes e cada um com seu respectivo histórico permite que os especialistas continuem com seu diagnóstico nas próximas consultas. Conclusões: A aplicação web agiliza eficazmente os processos relacionados com os registos médicos, o que contribui para o controlo e classificação da referida documentação.

Metastases to the breast from non-mammary origin are rare. The majority of these lesions appear as secondary manifestations of melanoma and lymphoma, followed by lung carcinomas, gynecological carcinomas, and sarcomas. There has been a steady trend of an increase in diagnosis of intramammary metastases owing to the current advances in imaging technology. Imaging features depend on the type of primary neoplasm and route of dissemination, some of which resemble primary breast cancer and benign breast entities. There are certain imaging features that raise the level of suspicion for metastases in the correct clinical context. However, imaging manifestations of intramammary metastases do not always comply with the known classic patterns. The aim of this review is to clarify these features, emphasizing radiologic-pathologic correlation and a multidisciplinary approach, since most cases are found in patients with advanced disease.

Objective: The objective was to identify the variables that affect the delinquency rate in banking and microfinance institutions, between the periods 2015 and 2020, for which panel data models were used, considering the information registered in the banking and financial institutions to the level of Peru. Method: The methodological design used is quantitative, not experimental, with a descriptive-correlational design, applying the analysis of the data panel for each financial institution (Multiple Banking, Municipal Savings Banks), to observe the behavior over time for the same individuals. Results: It was determined that the behavior of the delinquency of microfinance institutions is having significant effects on the delinquency of loans, and macroeconomic variables like microeconomic variables do determine delinquency rates such as provisions, efficiency of analysts, financial income, liquidity in national currency, growth rate of Gross Domestic Product, and the level of unemployment, both for banks and for municipal savings and credit banks, explaining the study variables in 84.30% in the banking system and in 48.95% in the financial system with respect to delinquency. Conclusions: Macroeconomic and microeconomic variables are determining factors for the level of delinquency in financial institutions.

Negative patterning tasks are a key tool to unveil the mechanisms by which stimulus representations are acquired-a central concern in Robert Rescorla's research. In these tasks, target stimuli are reinforced when presented individually (A+/B+) but not when presented in compound (AB-). The discrimination of single stimuli from their compound presentation is a challenge for theories of associative learning, because it cannot be explained by the simple accrual of associative strength. The present study examined the conditions under which mice learn this part-whole discrimination in olfactory stimuli using a novel instrumental methodology. In two experiments, reinforcement was contingent on distinct responses depending on whether a set of odor mixtures were presented in isolation or as a compound. Using C57BL/6 mice, Experiment 1 showed a mutual interference between learning a response to individual odors and learning a different response to those odors presented in compound. Using wild-type APP/PS1 mice (a control strain for a murine model of Alzheimer's disease), Experiment 2 replicated this interference and showed that it is stimulus-specific. These experiments show that the instrumental patterning task may not only complement Pavlovian negative patterning tasks but may also motivate its own questions on the representation of complex stimuli. The phenotypic changes in hematopoietic stem progenitor cells (HSPCs) with somatic mutations of malignancy-related genes in patients with acquired aplastic anemia (AA) are poorly understood. As our initial study showed increased CXCR4 expression on HLA allele-lacking (HLA[-]) HSPCs that solely support hematopoiesis in comparison to redundant HLA(+) HSPCs in AA patients, we screened the HSPCs of patients with various types of bone marrow (BM) failure to investigate their CXCR4 expression. In comparison to healthy individuals (n = 15, 12.3%-49.9%, median 43.2%), the median CXCR4+ cell percentages in the HSPCs of patients without somatic mutations were low: 29.3% (14.3%-37.3%) in the eight patients without HLA(-) granulocytes, 8.8% (4.1%-9.8%) in the five patients with HLA(-) cells accounting for >90% of granulocytes, and 7.8 (2.1%-8.7%) in the six patients with paroxysmal nocturnal hemoglobinuria. In contrast, the median percentage was much higher (78% [61.4%-88.7%]) in the five AA patients without HLA(-) granulocytes possessing somatic mutations (c-kit, t[8;21], monosomy 7 [one for each], ASXL1 [n = 2]), findings that were comparable to those (66.5%, 63.1%-88.9%) in the four patients with advanced myelodysplastic syndromes. The increased expression of CXCR4 may therefore reflect intrinsic abnormalities of HSPCs caused by somatic mutations that allow them to evade restriction by BM stromal cells.

Al 16 de julio pasado existían 108 vacunas en etapa clínica de desarrollo, 28 de ellas con estudios en fase III y 6 autorizadas por la Organización Mundial de la Salud (OMS) según datos publicados al 3 de junio (AstraZeneca/Oxford, Johnson and Johnson/Janssen, Moderna, Pfizer/BionTech, Sinopharm, Sinovac) (1,2). A la vez, las entidades regulatorias como la FDA (Food and Drugs Agency) y la EMA (European Medicine Agency) han autorizado para uso de emergencia las vacunas de los productores Pfizer/BioNTech, Moderna, AstraZeneca y Janssen (3) y en Chile el Instituto de Salud Pública ha autorizado 6 vacunas (Pfizer, CoronaVac, Astrazeneca, CanSino, Janssen, Sputnik). En las últimas semanas, en el contexto de la preocupante propagación de la variante Delta (B.1.617.2) del SARS-CoV-2, la compañía Pfizer solicitó a la FDA la autorización para la administración de una dosis de refuerzo (booster) con su vacuna BNT162b2 , luego de que el análisis interino de datos provenientes de la población vacunada en Israel mostrará una reducción significativa de los anticuerpos neutralizantes a los seis meses de completado el esquema primario, solicitud que generó debate en la comunidad científica. Hasta ahora los datos disponibles sobre eficacia de la vacuna BNT162b2 en escenario de circulación de variantes sugieren que continúa siendo altamente efectiva (4,5). Los estudios para definir la eventual necesidad de una dosis de refuerzo a diferentes esquemas primarios están en etapas de reclutamiento inicial en varios países.(AU)

Bluetongue (BT) is a viral disease transmitted by Culicoides (Diptera: Ceratopogonidae) to domestic and wild ruminants. Infections in cattle are mainly subclinical, but severe necrotic and hemorrhagic illness and death may occur depending on the strain of the virus and other factors; cattle act as a reservoir for the virus. Although the Ecuadorian coast has climatic conditions that favor the presence of the vector, there are few serologic or virologic BTV studies available. Manabí is a coastal province in which livestock farming is mostly implemented in the northern part. We conducted two studies to assess, for the first time, the presence of active BTV infections in Manabí province. We collected 430 serum samples from 38 randomly selected farms between March and July 2019 to perform BTV competitive ELISA. In addition, six seropositive farms were selected to place eight sentinel BTV-naive calves. All these calves were blood sampled and the presence of BTV RNA and antibodies was tested for by RT-PCR and competitive ELISA, respectively, once a week for 6-8 weeks until seroconversion was evidenced. A high individual seroprevalence (99%) was obtained, and all investigated farms had BTV seropositive animals. All sentinel calves became BTV viremic and seroconverted. The first viremia appeared after 2-5 weeks from arrival at the farm; they seroconverted 1-3 weeks later. We demonstrate for the first time that there is a high level of BTV circulation north of Manabí, with active infections on these farms. Integrated control strategies such as hygienic measures on farms to reduce midge populations would be advisable for the owners as mitigation measures.

ABSTRACT: Cerebrospinal fluid (CSF) leakage caused by skull base fracture represents high risks of bacterial meningitis, and a rate of mortality of 8.9%. Endoscopic endonasal repair of CSF leaks is quite safe and effective procedure with high rates of success. The aim of this study is to describe our technique for management of skull base CSF leaks secondary to craniofacial trauma based on the anatomic location of the leak. This is a retrospective case series of 17 patients with diagnosis of craniofacial trauma, surgically treated with sole endonasal endoscopic and combined endonasal/transcranial approaches with diagnosis of CSF leak secondary to skull base fractures. Seventeen patients met inclusion criteria for this study. Mean age was 46 years old. Most common etiology was motor vehicle. Early surgery was performed in 8 patients, and late surgery in 9 patients. The most common site of CSF leak was at ethmoid cells or at the fronto-ethmoid junction in 9 patients. Thirteen patients (76.4%) were treated only with endonasal endoscopic technique, and 4 (23.5%) with hybrid surgery, combining endonasal endoscopic and cranial bicoronal approaches with nasal and pericranial vascularized flaps, and nasal mucosal free flaps. Mean hospital stay was 23.7 days.The mean follow-up time was 25.6 months. When surgical reconstruction is indicated for CSF leaks secondary to skull base fractures, endonasal endoscopic techniques should be part of the surgical management either as a sole procedure, or in combination with classical transcranial approaches with high rates of success and low morbidity.
